Adaptations and Medical

 

Minor Adaptations

The council supports an adaptation programme to assist DDH tenants who may require alterations to their home in order to address a health or disability issue. The adaptations we provide include grab rails, stair rails, ramping, lever taps, key safes and other alterations tailored to meet the needs of the disabled resident.

 

The Process

The Medical Officer will arrange to contact you to obtain further information about your needs. This is called a single assessment process. We then follow this up with a home visit, if necessary. Home visits are arranged at a mutually convenient time, within three working days of the initial contact where possible. You will be kept informed throughout the process by phone, letter or e-mail.

 

Major Adaptations and Disabled Facilities Grants

The Council manages the Disabled Facilities Grant. In case where the Community Occupational Therapist advises that you require more extensive forms of adaptation, such as level access showers, ramping or extensions, you may be recommended to apply for a grant to help towards the cost of the work. The Disabled Facilities Grant is a means-tested benefit and you will be asked to complete a test of resources form for us to calculate whether you qualify for this service. You will be kept informed of progress throughout the process by phone or letter.

Applying to move under medical grounds

 

The Council’s Medical Officer is also responsible for assessing an applicant’s need to move under health or welfare grounds.  Please ask for a medical form when you request your housing application or click the link to download a form electronically.

 

The Medical Officer will assess each individual case upon receipt of the completed medical form. It may be necessary to visit the applicant at home. If further information is required to establish your level of need under medical or welfare grounds, with your permission, the Medical Officer will contact your doctor and/or other relevant person involved in your health and well being. All medical forms will be acknowledged and a letter sent to advise you of the outcome.

 

We do not advise you to obtain a letter from your doctor or other professional involved in your care. If the Medical Officer deems it necessary, we will write to them direct.  We are not able to request information from your doctor or other professionals in the absence of your signed permission.

 

Single Assessment Process

 

The single assessment process is a contact assessment referral system to enable staff to refer clients to any other appropriate agencies and ensure medical and social care services are involved where required to maintain management of health and welfare. With your permission, we share this information with other agencies as necessary, so that you are not continually asked to provide the same information.
